Photo and Testimonial Release

I hereby authorize Lincoln Savings Bank, herein referred to as “LSB,” and its affiliates and subsidiaries to publish, copyright, use, assign, sublicense and distribute my physical likeness, in photographic, video, or electronic reproduction form, in any promotional items or materials including, but not limited to art, illustration, advertising, merchandising, endorsement, trade, and all lawful purposes and on websites for marketing, social media, promotional items, or other activities relating to LSB.

I also authorize LSB and its affiliates and subsidiaries to publish, copyright, use, assign, sublicense and distribute confirmed comments made by myself, or on my behalf, about LSB, its affiliates, and its subsidiaries. I authorize that these comments may be used in any promotional items or materials including, but not limited to art, illustration, advertising, merchandising, endorsement, trade, and all lawful purposes and on websites, social media, promotional items, and other activities relating to LSB.

I agree that I will make no monetary or other claims against LSB for the use of my physical likeness or confirmed comments. I waive any right to inspect or approve the finished product where my physical likeness or confirmed comments appear.

This testimonial will remain valid for one year from the date signed below.
